Services provided include independent living homes and apartments; assisted living; memory loss care; home health, hospice and supportive (non-medical) care; geriatric care management; skilled (long-term) nursing care; short-stay (subacute) rehabilitation; outpatient rehabilitation; home medical equipment; education and retreat programs, etc.Located in six natural settings in southeast Wisconsin, with main campus situated above Big Cedar Lake in West Bend, WI and an additional campus in the heart of Elkhart Lake, WI. Home health and hospice services available to those living in Washington, Ozaukee, Sheboygan, Fond du Lac, Dodge and Waukesha counties.Home medical equipment (durable medical equipment) delivery / service area includes four southeast Wisconsin counties: Milwaukee, Ozaukee, Washington and Waukesha.The nation's first to provide specialized care for those facing Alzheimer's disease and related dementia / memory loss in 1976.
