While a child is experiencing rapid growth during their early years, well child visits (checkups) are necessary so preventative measures can be taken to help ensure your child stays healthy. During a well visit, your child will receive a medical exam to check for developmental milestones as well as their required vaccinations, according to age.
This Notice of Privacy Practices describes how we may use and disclose your Protected Health Information (PHI) to carry out treatment, payment or health care operations (TPO) and for other purposes that are permitted or required by law. "Protected health information" is information about you, including demographic information, that may identify you and that relates to your past, present or future physical or mental health or condition and related health care services.
A child’s school usually requires sports physicals annually before they are allowed to participate in their chosen sport. They are a great way to ensure your child is experiencing healthy development in areas such as lung development and spinal formation. Sports physicals help to identify conditions such as asthma that may affect your child’s ability to participate in a sport.
