Sinusitis
This is first and foremost a medical disease. The inflammation in the mucous membrane of most forms of chronic sinusitis is similar in nature to the inflammation that occurs in the airways of asthmatics. These conditions are best managed with appropriate medical therapy aimed at blocking the inflammatory response to inhaled particles, ingested sources of inflammation and eliminating secondary bacterial infections.
Rheumatology
Granulomatosis with Polyangiitis (GPA) is a vasculitis (inflammation of blood vessels) characterized by involvement of small to medium sized vessels and necrotizing granulomas (scattered spherical areas of tissue destruction). The cause is unknown although there are several theories currently under investigation. It is thought to be a type of auto-immune disease, where the patient’s own immune cells mistakenly begin attacking blood vessels and surrounding tissues. This disease was formerly known as Wegeners Granulomatosis prior to the American College of Rheumatology nomenclature change in 2011. The disease is known to affect blood vessels and adjacent tissue primarily in the ears, nose, throat, lungs and kidneys. One in 33,000 individuals are affected by this disease. The mean age at onset is 55 years old. Most of those affected (> 90%) are white. This disease can be fatal if untreated. Two main forms of GPA exist: 1) Limited (involving only the ears, nose and throat) and 2) Generalized (involving the ears, nose, throat, lungs and kidneys).

Anterior nose bleeds are often controlled with simple measures at home or with minor procedures in the doctor’s office or emergency room. Posterior nose bleeds usually require more care (balloon packing or electrocautery) and patients are often hospitalized for some period of time.

8. Consider an over the counter nasal steroid spray. When your nose has been subjected to allergies or other airborne irritants and nasal congestion is not relieved and prevented with the above measures then consider using an over-the-counter nasal steroid spray as directed for 4-6 weeks. These remedies are effective and relatively safe when used properly. 17,18,19,20 Be sure to aim the spray nozzle towards the internal sidewall of the nose rather than at the internal middle wall (septum). Do not use the steroid spray until consulting with your physician if you have frequent nose bleeds, prior nasal surgery or glaucoma. Discontinue the spray after 6 weeks unless you are under the care of a doctor who has recommended longer use.

Your nasal and sinus complaints will be fully evaluated by a fellowship-trained rhinologist (nasal and sinus sub-specialist). The latest technology and state-of-the-art medical therapy will be used to diagnose and manage your nasal and sinus condition. Your visit will include a full ear, nose, throat, head and neck exam. If necessary, video endoscopy of the nasal cavity, sinus drainage pathways and throat can be performed. Cultures are often performed for chronic or resistant infections.

Radiology
Patients with odontogenic rhinosinusitis present a unique challenge to the otolaryngologist. The diagnosis is frequently delayed. A dental source for the sinus infection is frequently not considered until after both medical and surgical therapy has failed to resolve the patient's symptoms. A primary cause for the delay in diagnosis is the failure of the radiologist and ENT physician to adequately assess dental pathology on CT imaging. In a 2010 retrospective case series involving 21 patients with known odontogenic rhinosinusitis, the initial radiology report failed to mention radiographic findings of dental pathology in 14 (or 67% percent) of the patients (3). Therefore, the otolaryngologist should have experience diagnosing dental pathology on CT imaging studies. In addition, there are symptoms which suggest an odontogenic source for the infection in the patient presenting with rhinosinusitis. First, the large majority of patients will present with unilateral symptoms. This means that the patient will complain of nasal discharge, cheek pain, dental pain or nasal obstruction primarily on one side. Another unique symptom to odontogenic rhinosinusitis is a foul smell noted by the patient. In a review of 76 patients with known odontogenic rhinosinusitis, 33 or 43% reported a foul smell (3,4,5).

Safety during sinus surgery cannot be over emphasized. The anatomic relationship of the nasal and sinus passages to the brain and eye creates the potential for significant, albeit rare, complications. Image-guided endoscopic sinus surgery allows for highly accurate visual updates during your sinus surgery. In IGS, your CT scan images are digitally linked to a computerized image-guidance system displayed on an operating room monitor via a sophisticated software program. Your CT scans are coupled to the surgical instruments by an electromagnetic tracking system. This gives the surgeon the ability to navigate, in real-time, the surgical instruments within the diseased sinuses, and correlate their position with vital structures while watching the monitor.

Radiation Therapy
Surgery combined with radiation therapy (and in some cases, chemotherapy) is the standard of care for esthesioneuroblastoma. Patients with this malignant tumor are best served by having an evaluation with an otolaryngologist (ENT physician), neurosurgeon, radiation oncologist and possibly medical oncologist.

Dr. Rehl is a board certified Otolaryngologist-Head and Neck Surgeon who founded the Arizona Sinus Center in 2007. He is a Past President of the Arizona Society of Otolaryngology-Head and Neck Surgery and the current Section Chief of Otolaryngology-Head and Neck Surgery at Banner University Medical Center-Phoenix. Dr Rehl is a Clinical Assistant Professor at the University of Arizona College of Medicine-Phoenix and a Fellow of the American College of Surgeons. He has been repeatedly selected by his peers to Phoenix Magazine's Top Doctors list (2009, 2010, 2015, 2016, 2017, 2018, 2019, 2020).

An Otolaryngologist is a physician trained in the medical and surgical care of head and neck disorders. Commonly referred to as an “ear, nose and throat doctor,” an Otolaryngologist must complete 4 years of medical school and then at least 5 years of head and neck surgical and medical residency training to be eligible for the American Board of Otolaryngology exam. To become board certified, the otolaryngologist must pass a written examination as well as a rigorous oral exam. Board Certified Otolaryngologists have demonstrated the required competency in otology (ear disorders), rhinology (nasal and sinus disorders), allergy, head and neck surgery (thyroid and parathyroid surgery, throat cancer, salivary gland surgery, sleep apnea surgery, neck tumors), pediatric ear, nose and throat disorders, laryngology (voice and swallowing disorders) and facial plastic surgery. Otolaryngologists serve as community resources for public health issues including hearing impairment, oral and skin cancer prevention, obstructive sleep apnea and air quality.

For patients with nasal or sinus cancer, we often work closely with our colleagues in oncology, neurosurgery, head and neck cancer surgery and prosthodontics to provide outstanding multi-disciplinary care. Our surgeons at the Arizona Sinus Center have experience treating the following nasal, sinus and anterior skull base tumors: inverting papilloma (Schneiderian papilloma), juvenile nasopharyngeal angiofibroma, hemangiopericytoma, ossifying fibroma, osteoma, fibrous dysplasia, hemangioma, schwannoma, meningioma, pleomorphic adenoma, pituitary adenoma, lymphoma, esthesioneuroblastoma (olfactory neuroblastoma), adenocarcinoma, squamous cell carcinoma and melanoma.

Nosebleeds are quite common in Arizona. Our arid climate dries the delicate mucous membrane lining the septum (middle wall) of the nasal cavity, allowing it to crack and tear the small, superficial blood vessels underneath. In addition, chronic or improper use of nasal sprays can cause irritation or ulcer formation on the septum leading to bleeding. High blood pressure, bleeding disorders and blood-thinning medications (Plavix, Aspirin, Coumadin, etc.) increase the risk of nose bleeds. Most nosebleeds are a nuisance but some can be frightening and very rarely, they can be life-threatening.
Fungus
The sinuses are hollow areas in the skull occupying the space above, between and below the eyes. They are connected to the nose and can get inflamed or infected leading to drainage, congestion, decreased smell, fatigue and pain. The underlying cause of the inflammation may be air pollution, allergies, polyps, abnormal internal nasal and sinus anatomy, reactions viruses, fungus and bacteria or swelling inside the nose.
Lesions
Odontogenic rhinosinusitis is defined as sinusitis induced by a dental lesion. The common etiologies include periapical and periodontal disease, odontogenic lesions and iatrogenic and traumatic causes. Iatrogenic causes include complications of tooth extractions, implants, sinus floor augmentation, osteotomies and other orthodontic surgery, and a foreign body reaction to either the root canal sealant or filling material used in root canal therapy. A significant amount of literature is dedicated to complications associated with dental implants and sinus lifts. The reported incidence of rhinosinusitis resulting from sinus augmentation is 0-27% (1). However, when strict criteria are used to define rhinosinusitis, the incidence is 4.5%. Approximately 1/3 of these patients will progress from acute to chronic rhinosinusitis. Chronic rhinosinusitis requiring surgical intervention from sinus augmentation is reported to occur in 1.3% of patients (1). Sinus inflammation or swelling on pre-augmentation CT imaging is a statistically significant risk factor for post procedure acute or chronic rhinosinusitis (2).These are patients who would benefit from a ENT evaluation prior to their “sinus lift” procedure.

Allergies
This is the “million dollar question” that rhinologists, allergists, immunologists and ear, nose and throat doctors all over the world are trying to answer. Remember, sinusitis means inflammation of the sinus mucous membrane. The cause for chronic sinusitis is probably multifactorial. It seems that many patients with chronic sinusitis actually have an overactive immune response to inhaled foreign particles. This can be in the form of a “true allergy” confirmed by blood or skin allergy testing or an “allergic-like” response (when the problem clinically behaves like an allergy but the allergy tests are negative). Some researchers believe the chronic inflammation is due to certain bacteria residing in the nose and sinuses which cause a chronic infection that is resistant to standard treatments because they have formed a sticky protective blanket (biofilm) shielding the bacterial cells from antibiotics, nasal rinses and sprays. Others believe that bacteria in the nose and sinuses secrete a toxin ultimately leading to inflammation in the sinuses. Multiple reports also point to inhaled fungal spores, and the body’s reaction to them, as a cause for persistent inflammation. Some patients may have other medical conditions predisposing them to chronic sinus inflammation or infection (Cystic Fibrosis, Wegener’s Granulomatosis, Sarcoidosis, HIV/AIDS, Kartagener’s Syndrome, immunodeficiencies). A patient’s unique nasal and sinus anatomy (narrow drainage pathways, deviated septum, extra obstructing sinus cells) may also result in inefficient sinus mucous drainage and subsequent recurrent or persistent inflammation.
