- Welcome to Stephen A. Cook, DDS, MSD! We are a locally owned and operated dental clinic specializing in periodontics and dental implants. We offer services that include dental implants, dental hygiene, dentures, crown lengthening, bone grafting, tooth extraction, and much more. We work together with you and your family dentist to help you establish and maintain lifetime dental health. Call us today. About the Doctor: Dr. Cook attended Purdue University for undergrad study in physics and pharmacy. He received an early admission to Indiana University School of Dentistry at the Med Center in Indianapolis and graduated with honors in 1978. He subsequently spent four years in the United States Air Force as a dental officer. After completing his service in 1982, he began a two-year post-graduate residency program, earning a Certificate of Specialization in Periodontics, as well as a Master of Science in Dentistry degree in June, 1984. He opened his periodontal and implant surgery practice in Lafayette in September, 1984 at its current location and has been pleased to serve the periodontal and implant-related needs of patients in the Greater Lafayette area for over thirty years.

Following implant surgery 23 years ago my goal has been to maintain my teeth and gums for my lifetime. I am now 80 and continue to do well thanks to Dr Cook and the Hygienist's thoroughness.
I like the personal service that he gives,like personally calling his patients to see how they are doing, it means a lot and shows that he cares about his patients. Also the friendliness of the staff and thier flexibility in working with the patients schedule. Would highly recommend them.
Dr. Cook takes the nerves out of going to the dentist. He always explains what work he is doing and is great about disclosing what to expect after a dental procedure.
They get you in to your appointment on-time and the staff is always personable and kind.
Dr. Cook always make you feel the security of being in skilled hands.
